Please telephone or use the link on our website if you have to cancel your appointment.

Appointments are often in short supply so please ring and let us know if you cannot attend. It means that the appointment can be given to somebody else.

New Patients

New patients are required to complete a registration form and a questionnaire about your health. This ensures that the GP’s know as much as possible about your health until your records are forwarded from your previous doctor, which can take several weeks. All patients’ records on file or on the computer are confidential. Information will not be given to others without your written consent and information kept on the computer is protected under the Data Protection Act (1984). However it is sometimes necessary to share your information with other health care professionals.

If you require a carer or family member to obtain test results or other information regarding your health you will need to sign a document giving us permission to release these details.

New Patient Registration

Registrations are taken on Monday to Friday between 12:15pm 13:15pm

Documentation required:

Proof of ID

Either:

  • Passport with valid visa
  • EU ID card
  • Home Office Immigration papers only if accompanied by ID card
  • Driving licence if you were born in the UK
  • Birth Certificate if born in the UK and have no photographic ID

Proof of address

Dated within the last three months.
Either:

  • Utility bill
  • Bank statement
  • Mobile telephone bill
  • Tenancy agreement
  • Council tax bill

Information required

Previously registered in the NHS

  • Your last address
  • Where you were previously registered with a doctor
  • Your place of birth
  • Any previous surname

Not previously registered within the NHS

  • Date of entry into the UK
  • Place of birth


Pre-Registration with the practice

If you wish to pre-register you will need to complete both the forms below.  Click on the link below to open the forms. When you have completed all of the details, click on the "Send" button to mail your form to us. Repeat this process for the health questionnaire. Completing these forms will save you time when you come to the practice to register.  You will need to sign these forms in person and produce your ID.

Pre-registration Form

You are asked to fill out a health questionnaire because it can take a considerable time for us to receive your medical records. 

When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
